OpenCVのバージョンは、3を使用してください。4だとPyinsatllerでパッケージ化したときにエラー吐きます

Note

pyinstallerで.app化するときは、app/DEBUG.pyのDEBUG=TrueをDEBUG=Falseにしてください。
