fhir.ch: Implementation Guide for the meta data specified in the framework of Annex 3 and
Annex 9 of the FDHA Ordinance on the electronic patient record in Switzerland.

The IG also contains VS form Annex 5

in the ig itself we have it already generalized, but limited to the annexes:
