Summary

Here is the enormous, long-awaited refactor of names for chain items:

Element -> Record
Header -> Action
Entry remains unchanged

All derivative names are similarly changed. I tried to find a few awkward points of grammar due to the new names, but didn't look too hard.

TODO:

• CHANGELOG(s) updated with appropriate info
• Just before pressing the merge button, ensure new entries to CHANGELOG(s) are still under the UNRELEASED heading
