Dynamic fields settings page

When toggling "Send confirmation email" or "Overwrite the e-mail template" in the settings page the corresponding fields hide/show, which is great. But if these switches are already off when the settings page is loaded the corresponding fields show by default (which they shouldn't, and toggling the switches off and on 'fixes' it).

I realise this is a small nitpicking issue, but it would be nice if those states were checked in the init.

@SayChi good catch. It looks like the Lightswitch states went awry.
