The most critical aspect is not losing "never indexed" for proxies. This will probably require an additional field on HeaderValue to track the information.

HeaderValue contains an is_sensitive flag which is sufficient for the requirements. Any further integration w/ HPACK encoding details should be handled at the h2 library level.
