Hi. Where can I get or see a proper sample CSV file so I can upload a contact list? Thanks

CSV file format like following

"923330000000","First1","Last1","email@domain.com","street address","custome value 1","custome value 2","custome value 3","any comments will be here"
"923331111111","First2","Last2","other@domain.com","street address","custome value 1","custome value 2","custome value 3","any comments will be here"

or

923331111111
923331111111
