IPython sphinx directive promptin requires linenumber

Can we remove the requirement for %d to be specified in ipython_promptin? I assume this also applies to ipython_promptout as well although I haven't tested it.

I'm not sure, but it seems like it should be a simple fix in the code to replace promptin % lineno in several parts of the script with a check for %d in promptin. If there is %d, use the current code, if there isn't just use promptin.
