Believed to be README-complete.

With the merging of PR #4, this demo app is now believed complete per the list of features in the README. These include a "simple" demonstration of

1. A model-based form that
2. Uses a form builder, not raw HTML form tags, and
3. Fortitude widget-based views, operated from
4. Modern, simple-endpoint controller action methods that use
5. Form objects to wrap the model instance used.

I'll bump this to 1.0 on or about 1 January 2016 unless there are issues and/or PRs filed. Thank you for your support!
