Feature request: use ini variables in default cover definition #25

It's easy for me to hack myself a little cover image script and use that as the 
URL for the cover. Unfortunately variables like ${title} are not expanded. 

If that's easy to fix, it would be nice to have.

That's actually a really clever idea.  

By using the same templating as output_filename, a person could do cover by 
title, or by site, which has also been requested.

Thanks for the suggestion.  I'll try to get it into the next version.
