Prepare package for release

This issue is here to track (and remind of) the needed changes that are required to the package in order to be released via Github and NPM.

• Add correct folders to the package.json entry (dist was changed to lib)
• Add pre / post install hooks
• Update descriptions, titles, contributors, license
• Update readme.md with colony logo / header
• Fix Improve warning/error message imports #16

It seems the es modules build is broken. It outputs packages in the commonJS format.

Let's also add a prepublish hook to check and build the necessary files to publish it to npm
