Auto-Generate Presenter Outline

HydeSlides should provide support for auto-generating a presenter outline based on chapter order defined in the slidedeck markdown. The outline content would be derived from the per-slide notes content.

• Shortcut key for launching from slidedeck (N)
• Jekyll layout for presenter outline page
• Means of parsing-out per-slide note content

Presenter outline/table-of-contents evolved and became #6. Presenter notes pop-out from T-of-C under development.
