Restores the ability to provide a WHITELIST environment variable, which was accidentally broken in #6.

@struan @hecticjeff I further fixed the parsing of WHITELIST, added a bunch of tests, moved files around, and released 0.0.2 on npm https://www.npmjs.org/package/image-proxy

The version with just the two recent pull requests (before doing all the above) is tagged 0.0.1 in git, but not released on npm
