ATINIT for Host MLI support under ProDOS 8.

host.mli.po image includes ProDOS 2.4.2 and exerciser.system for testing.

host.fst.* includes FST and driver for GS/OS

Updates:

1. Post volume change events when a file is created/modified/deleted.
2. move the .host device check from the fst startup call to the application call, as needed.
3. experimental boot stub code to boot from the host.fst.
