A few adjustments with tons of permutations

Same planners, updated representation:

• todos are killed in favor of notes;
• notes index as a list, similar to TOC;
• all permutations for these options:
  • dotted or lined pattern for notes;
  • Monday or Sunday as the first day;
  • absence or the presence of the little calendar on daily pages;
  • 24 or 12 hour format on the daily page;
  • planners for 2021 and 2022 years.
