Some outputs are not showed.

Figure out how to read DXGI_MODE_SCALING_*
Figure how to handle flag DXGI_ENUM_MODES_INTERLACED
Figure how to handle flag DXGI_ENUM_MODES_SCALING
Figure if "DisplayFixedOutput" is requireed or not
Figure how to handle flag DXGI_ENUM_MODES_DISABLED_STEREO (WINDOWS8+)

Might be related: EDS_RAWMODE on EnumDisplaySettingsExW for some flags
