This release fixes breaks during the build of documentation and the installation of reports.

This is the first github-hosted release of LifeLines.

The critical fix included in this release is a fix for broken UTF-8 support, which has been broken for way too long.

The remaining changes are largely infrastructure related:

• moved project hosting from sourceforge to github
• moved web pages from master:/web to gh-pages:/ (to align with github web hosting requirements)
• upgraded autotools and docbook to modern levels
• improved release scripts
