[UX][Create Identity] This design of Create Identity is not good enough for user experience

*Check list:

1. Signature text area:

• Type of description should only accept Html option, no need to show Plain text/Html option
• The padding left of inputted text must be increased a bit
• The signature text area should be smaller. Beside that, we should add [Show/Hide] button to decrease the signature text area. It is too big right now which made us can't handle the second issue which mentioned bellow.

2. Cancel/Create button

• These button should be move to upper a bit which closer with Set as default identity checkbox
