This has happened at least twice in a row for me, so perhaps it's something more than just chance? The finals match in a 3-round draft between the two undefeated (2-0) players doesn't launch properly. At least one player is just stuck in the tournament lobby and the game window doesn't launch for them.

Reproduced this today. My opponent was unable to join our match.
