Token choice and transaction form template #5

We need a form to

Choose a token from a dropdown with token name and icon
enter an amount in a form field
a button to confirm the transaction (we'll have different labels for different transaction types)

A lot of the necessary building blocks should be in the code (i.e. the dropdown with tokens should be in the trading window code).
