Fixup 4.0 change log (kokkos#6015)

* Fixup 4.0 changelog: `KOKKOS_ACTIVE_EXECUTION_MEMORY_SPACE_*` macros oversight was in release 3.6 (not 3.2)

* Fixup 4.0 changelog: drop deprecated entries duplicated from 3.7 changelog

* Mention removal of deprecated code 3 in change log

* List removing deprecated code 3 under incompatibilities
