The CSS and markup around the search bar is the remnant of a number of refactorings and adjustments, both pre and post bootstrap. Redo the markup to better match current bootstrap norms. This fixes a number of layout problems.

This revises the CSS and markup for the search box and home page to closer match normal Bootstrap 3 norms, and fixes some layout issues along the way.
Fixes #1605 and #1606.
