Create standard, optional parameter across plugins to indicate the unique community identifier on a platform

For instance, in the Slack plugin we just re-use Slack's name for the community identifier, team id, but we want something standard across all plugins.

Currently in the identity pr #119 I use community_platform_id but we could choose a better or less verbose name.
