Generated PDF is missing page

When running the following command the first input file specified is not included in the output PDF.

htmldoc --footer ... --format pdf --left 0.75in --outfile output.pdf --right 0.75in --webpage input_file1.html input_file2.html input_file3.html input_file4.html

(input files and output PDF file attached)

The PDF has 4 book marks created (one for each file), however there are only 3 pages in the PDF. From experimentation it appears that that it is always the first file that is dropped.

htmldoc --version
1.8.27

Tested on two separate MacBook Pro
sw_vers
ProductName: Mac OS X
ProductVersion: 10.6.8
BuildVersion: 10K549

The problem is also occurring on a server running
Gentoo Base System release 1.12.11.1

After additional testing it appears that the
<!-- PAGE BREAK -->

Appears to be causing the issue. Removing this comment seems to solve the issue.
Is this expected?
