Evaluation Timeouts

Visual Studio supports aborting an evaluation after a certain amount of time has passed.
Visual Studio passes an additional field in the arguments of the evaluate, variables, setVariable, and setExpression requests.

If the debug adapter supports timeouts, it should abort any request that exceeds the timeout by sending a response with success set to false.
