Run cargo fmt. Update the #[debugger_test] attribute to ensure the debugger always quits after running the test function. Force quit the debugger if the test has been running for too long waiting for the debugger to detach.

Update and cleanup API and implementations to support Rust 1.45.0 as the MSRV. Also update the CI pipeline to test against Rust 1.45.0 to ensure regressions are not introduced.
