This is a bit of a pain and a lot of paperwork, but it's done. FTC has a good form on their website where you can fill in all this information, but all that does in the end is fill out a PDF for you that you could have downloaded in the first place. It does make a record with the FTC though, so that's something. Fun fact, if you download the PDF, you get a different version of it than if you fill in the form.
Anyway, once it's all filled in, you need to get the police, a notary, and/or a witness to sign it, and you need to sign it in the presence of the police. In Berkeley this means sitting in the waiting room until somebody can help you, which could take hours since somebody from the field has to come to the station for your piddly form -- they're not inclined to do this.
The form is only needed to create seven year blocks with the credit agencies, so I've foregone the police signature and simply sent in the form signed by a witness. It's not clear whether that's enough, but we'll soon find out.
