We probably don't want --at least initially-- regular users to be able to create organizations directly. So we need an option for them to make corresponding request. Designated admin user can then (upon some form of notification) review the request.

@graybeal Can you comment on how important this is? Perhaps this could actually be handled outside of the ORR. By "outside of the ORR" I mean that people would "simply" get in touch with an admin to request the creation of the desired organization.
It's useful in the broad sense, but maybe can be replaced by a static "Please email {someone} to have your organization added." Is there an email address for each site administrator?
Yes, an ORR instance has a primary admin (and any number of "extra" admins). The email could be sent to the primary admin.
In terms of UI, perhaps have a dropdown like this (actually with two options because they are related):
Organization:
Request creation of organization
Request joining an organization
Besides capturing relevant information, some text can be included indicating that an email will be sent to the corresponding administrators for review (site admin in the 1st option; admins of the target organization in the 2nd option)..
