Remove all references to the Disconnect category, as this category has been removed.
Remove all references to the outdated fastblock lists, which still contain the deprecated Disconnect category.

This PR removes all references to the Disconnect category, as this category has been removed since mozilla-services/shavar-prod-lists#110.
It also removes references to the outdated fastblock lists, which still contain the deprecated Disconnect category, as discussed in mozilla-services/shavar-list-creation#121.
This should be merged before mozilla-services/shavar-list-creation#121.
