In the contact information, the name/email input fields should be required unless the "File report anonymously" checkbox is checked.

When the user is not authenticated, name and e-mail are required without the checkmark

@willdurand Forgot to add: the name and e-mail fields are not visible yet in admin , but there's an issue about it
