Users still occasionally able to submit to phantom agencies with exact same name

Going through processing requests, I saw this occasionally crop up mostly with Department of Defense, Office of the Secretary of Defense and U.S. Secretary of State. I'm deleting those phantom agencies, and if the issue keeps cropping up I will try to replicate by creating two identically named Test Agencies, one of which is approved and one rejected.
