Rework how network families are defined with generic sock addr #962

Previously, we defined NGTCP2_AF_INET and NGTCP2_AF_INET6 to our chosen values, but they are arbitrary values, and hard to reason about.

With this commit, if NGTCP2_USE_GENERIC_SOCKADDR is defined, NGTCP2_AF_INET and NGHTTP2_AF_INET6 should also be defined externally.
