Hi, @sk9331657! Welcome and thanks for the pull request.
Modifying these templates is not to be done lightly. Adding another question to the issue template is adding another obstacle to people reporting issues. This does not appear to solve any particularly important issue that has created problems for the project. I don't think we should do this.
