Port to Zurb Foundation

0.9.3 (2014-01-02):

• Use Zurb Foundation to control the layout of the generated pages.
• opam doc -n <pkgs> runs without a server now (instead of --no-server).
• opam doc-serve will default to the OPAM root now.

Handle duplicate module names better

Use rsync instead of `cp --parents`, which is a GNU-cp-only flag

Portability and more permissive about errors

• Explicitly invoke bash since some scripts use bash-isms.
• Do not actually change the current compiler switch when using opam doc.
• Make opam-doc more relaxed about errors.
