https://github.com/siteadmin/inferno/blob/2604511d2835a5c6914794942da868ca940a9c22/lib/app/utils/validation.rb#L42 looks at just the first Coding, but http://www.fhir.org/guides/argonaut/r2/StructureDefinition-argo-vitalsigns.html really just requires that any coding of (say) {"system":"http://hl7.org/fhir/observation-category","code":"vital-signs"} be present.
