Param Structure update

In response to feedback over the last few months, the proposed changes are:

1. Default is set to either (paramStructure should default to "either" #270 paramStructure should have an "either" value #190)
2. document root has a paramStructure field which overrides the method default. (should paramStructure be global? #102)
