Magento Auto-Assign Customer Group

Based on URL paramter values collected by Shopix_StoreUrlParamOrder, assign customers to customer groups.

Add number of fields to customer group entity:

1. Group Name
2. Group Title
3. Group Owner
4. PDF Code
5. URL Code
6. First Name
7. Last Name
8. Role
9. Email Address
10. Phone Number

Extend Customer Group edit page and grid.

Display customer group in Newsletter subscribers grid.

Pass group attributes to MailChimp subsribers' data (via Ebizmarts_MageMonkey) so that they can be used as MailChimp merge vars.

Depends on Shopix_StoreUrlParamOrder.
