Reformat footer for editables to improve speed

Now primary appraisal of one record (screened,
specialty tagged, and title written)
takes around ten minutes.

To move on, I need to reduce it to one minute
or two minutes more realistically.

So I moved all Steps from Step 3 on
to the bottom of the page, because
I don’t use them often.

Also, I grouped specialties by my Nosology Memo v14
classification to speed up specialty
recognition in the list:
