Better Document each algorithm step

The algorithm is currently a transliteration of the pseudocode from the Guttmann paper. It's very hard to follow unless you have the paper side-by-side.

It would be better to provide some explanations inline with the code. It would also be good to include the algorithm section markers from the paper in the comments as well.
