Text field for open responses

Hello!
We aim to use the tool as a feedback tool for teachers and would like to involve them in its design process. To facilitate this, integrating a text field into the tool for teachers to propose item suggestions would be fitting.

Would it be possible to incorporate an open response text field for suggestions?
