Pouch will create an empty doc if necessary on putAttachment #2

As of issue #515 on pouchdb, api.putAttachment will create an empty doc on which to put the attachment automatically, which means that the code to do that at https://github.com/nick-thompson/pouch-server/blob/master/index.js#L221 is unneccessary.
