Migration from Bitly to Pusher

This is a work in progress PR to adopt this repository as an official hard fork of https://github.com/bitly/oauth2_proxy

Migration checklist:

• Add notice to Pusher fork that this is a hard fork
• Add notice to Bitly fork that the project is deprecated and list significant maintained forks
• Set up CI for Pusher repo for linting and testing
• Set up automated builds of master/tags on Quay
• Add link to Quay in README
• Lint the repository
• Create issue and PR templates
• Create CODEOWNERS defaulting to Pusher people
• Find volunteers to help maintain project (and work out how to manage them)
• Add CONTRIBUTING.md
• Start process for migrating PRs
• Create v3.0.0 release

With KubeCon last week I didn't get much of a chance to work on it and have a lot of wrapping up to do at work this week before the holidays. I may get time over christmas to work on it but most likely I will pick the work up in the new year. Apologies for the delay.
