Unfortunately, the source you have linked to no longer exists. Can you give another source for your towgs84 parameters? Unfortunately, the GDAL that comes with OSGEO4W and Ubuntu still uses the 3-parms-version by NIMA, which is surely less accurate. But you might end up with some nasty offsets if QGIS runs GDAL commands in the background.
I checked the EPSG database, which is the ultimative source for GDAL parameters. They offer different values for the Czech Republic and Slovakia, but none of them meet yours.
