Unless the image is very small, the density map is calculated at a lower resolution than the full image.
The hotspot calculations are 'rasterized', meaning they work on pixels in the lower resolution density map.
This imposes a limit on the accuracy.

Once the hotspot is created, the object counts QuPath makes inside the annotation are no longer limited by the density map resolution.
This means that the number of objects counted in the hotspot may not be identical to the value in the density map.

It also means that slightly shifting the hotspot (by a distance lower than the density map resolution) can sometimes give a slightly higher value.
In this way, the command really provides an efficient estimate of hotspots, rather than precise locations from an exhaustive search -- but any differences can be expected to be small.
