SetContentTypeFormUrlEncoded fixed

Pada method SetContentTypeFormUrlEncoded(), value untuk konten type tidak ada "application". Existing content-type = "x-www-form-urlencoded", seharusnya content-type = "application/x-www-form-urlencoded". Ini menyebabkan form tidak dapat diparse.
