feat(service): Add support for multiple Instagram accounts.

You'll need to login via `http://localhost:3006/auth/instagram` to obtain an access token. It's still backwards compatible so it'll drop back to using the `INSTAGRAM_ACCESS_TOKEN` environment variable if it doesn't find any auth tokens in the DB.

Very spaghetti-like, but I'll tear it apart when I hit another OAuth integration (probably randytarampi/me.photos#9).
