Accessibility issues with HTML file upload control

Steps to reproduce the problem

In the File pane, click the Upload button

Describe the problem in detail

A standard HTML <input type="file"> is used for the file upload (this is necessary), however these controls have several accessibility problems, such as not exposing names of the selected file, or even of the button itself, to screen readers. Still marginally usable given the surrounding context, so not a blocking issue.
