What are users / admins supposed to do when they see this message:

It smells like a bug but it's not clear if it's because of wrong configuration or something else failed.

It smells like a bug to me too. I imagine the error message "should never happen" suggests that nobody believed this scenario was possible, and it apparently is happening regularly for you.
