After launching the app, no extension is installed in Library/Extensions, so quick-looking an md file does not use QLMarkdown for rendering.

Running Big Sur 11.1 on a 2014 MBP.

Try to move the app in another folder (best is /Applications). Or put in the trash and then back.
Then check system preferences/Extension/Quicklook extensions

OK, it's there now; I think I was looking in the wrong place. Thanks. However, perhaps I misunderstand what this app/extension will do. I expected the quicklook window for md files to look similar to the app's window. If so, I still only see a plain QL window:

The app window is used only to configure the quicklook preview (choosing which style, extensions and options to use). So you don't need to use it anymore.

The QL preview shows the formatted markdown file instead of the source code that composes it. If you have other applications that implement a QL preview for the .md format it is possible that they are used instead of this app.

I believe that from System preferences / extensions / quicklook you can reorder the list in order of precedence.
