Added sub-apps support

Now path helpers inside nested apps returns correct urls:

// bundle express app as nested
app.use('/nested', nestedApp);
// was
nestedAppRoutes.paths.someRoute() // /some/route
// now
nestedAppRoutes.paths.someRoute() // /nested/some/route

Singleton resources
