Show database in name of execute write thread

I noticed that the https://datasette.io/-/threads page sometimes has more than one thread with a name like Thread-1 (_execute_writes) - that's because each attached database that has had at least one write gets a dedicated write thread.

Adding a thread name will make this more obvious and easier to debug.
